Order form

After selecting your shipping method, the next step is to complete the order form.

First, review the item along with the prices for Shipping, Tax, and the Total amount that will be charged to your credit card.

Next, Fill out the form. The form is pretty self explanatory. The first section is the Bill To Information. This is the information regarding the purchaser (YOU). The only thing to keep in mind is that there are certain fields which are required and are marked with *.

Here is a list of the required fields for the Bill To Information:

• First and Last Name
• Complete Address
• Email Address
• Phone Number (not necessarily your home phone number, but a phone number where we can reach you)
• Credit Card Information (type, card number, and expire date)

There are no required fields for the Ship To Informaion. This section is for the person you wish to send the item to. If you wish to ship the item to the same address in the Bill To Information, just check the box that reads Check box if same as above.
